The date given is that of the presentments exhibited at the visitation at Lichfield. The entry is enclosed by a brace on the right, and space at the right side of the brace is blank, as is true of four of the six cases on this page (the exceptions have a word or two of scattered notes). The marginale, 'ex', presumably stands for 'extrahatur,' ie, 'copy this out,' and directs the clerk of the court to copy this presentment into the court book, where the proceedings and disposition of the case would be later added.
