The date given is that of the proceedings of the court held in Lichfield Cathedral at the metropolitan visitation of the diocese of Coventry and Lichfield. Court information is taken from the acta paragraph on p 1; another acta paragraph occurs on p 12b, but this was a loose paper before the rebinding and repair of the volume, and it may well not be correctly placed. It gives the date as 25 August and the place of proceedings as the home of the chancellor, in the presence of Simon Martin, notary public. The phrase 'vt supra' presumably refers to the previous (unrelated) case, in which the defendant is ordered to make public confession in his own parish church on the following Sunday and to make a copy of the schedule of penance. Although not specifically mentioned, the need to certify compliance is implied by mention of the copy of the schedule, because such certificates were normally written on the dorse of the copy.
